WHAT CONTACT INFORMATION IS REQUIRED?Updated a year ago

The following information is required by law to be on all customer labels:

• Must include the phrase “Formulated for” or “Distributed by” on the first line of their contact information.
• City, State, and Zip Code is required, at minimum.
• Phone number is required. 

In addition to the required contact information, the item number is required to be on the label and cannot be changed. You may change the placement of this number, but it must remain visible.

You can also use a QR code using our generator in the design studio. However, we will still require the contact information listed above to be on all labels.
